InForm Imaging (“InForm Imaging,” “we,” “our,” or “us”) provides private diagnostic imaging services focused on extremity magnetic resonance imaging (MRI). Our services are offered to individuals seeking imaging and to healthcare providers referring patients for imaging.

The diagnostic imaging services provided by InForm Imaging are intended to support clinical assessment and decision-making. Imaging results and radiology reports:

  • Do not constitute a medical diagnosis on their own

  • Should be reviewed and interpreted by a qualified healthcare professional

3. Eligibility and Use of Services

Cancellations and Changes

  • Appointment changes or cancellations must be made within the timeframe communicated at booking

  • Late cancellations or missed appointments may be subject to fees

5. Fees and Payment

Fees for imaging services are disclosed prior to booking.

Payment terms:

Payment is required prior to or at the time of service unless otherwise agreed

All fees are subject to applicable taxes

Services are privately paid and are not billed to provincial health insurance plans

InForm Imaging does not guarantee reimbursement from private or extended health insurance providers.

6. Referrals and Results

While a referral is not required to book an appointment, imaging results are intended to be shared with a qualified healthcare provider.

Radiology reports are prepared by licensed radiologists and delivered within estimated timeframes. Turnaround times are not guaranteed and may vary due to clinical or operational factors.

7. Website Use

You agree not to:

Use the website for unlawful purposes

Attempt to gain unauthorized access to systems or data

Interfere with the operation or security of the website

Website content is provided for general information purposes only and may be updated without notice.

8. Intellectual Property
